About FormKiosk

FormKiosk is built for gathering feedback while a customer is still at the counter. You build a branded form with nine question types, conditional logic, your logo and colors, then publish it to get a short link and a printable QR code that never changes. A tablet can be parked by the till and the form resets itself for the next person, so it collects all day.

Results show completion rate, average rating per question and each response as it arrives, with a CSV export. The question types include a star rating, an NPS score and long text. The site names shops, cafes, pharmacies, restaurants, salons, car services, hotels, gyms, bakeries, clinics, dentists and events as typical users, and includes a live demo form. A free trial is offered, and pricing is on a separate page.
