CrispForms

CrispForms is a web form builder for people who care about how forms look and whether respondents finish them. One editor produces two kinds of form: conversational forms that show one question at a time, and single-page forms that show everything at once, optionally split into steps. A form can be switched between the two without losing its questions, logic, or responses.

Forms support conditional logic, scoring, and more than thirty themes, and responses can be exported in presentable formats. You can start from templates, describe the form you need in a sentence and have it written for you, or build it manually. Conversational mode includes keyboard shortcuts, transitions between questions, and a progress indicator. According to the vendor, the free plan includes unlimited forms and a monthly response allowance, and payment is only needed to remove the CrispForms badge.
